Configure SharePoint connection and defaults
⚠ SharePoint integration requires Microsoft Graph API authentication. Once you provide your SharePoint URL below, the app will prompt users to sign in with their Microsoft account when submitting data. No admin privileges are required.

SharePoint / Excel Storage

Provide your SharePoint site URL and the path to the Excel workbook where data will be stored. The file will be created automatically if it doesn't exist.

Teams App Sideloading

To install this as a personal app in Microsoft Teams without admin privileges:

  1. Host this HTML file on any HTTPS URL (SharePoint, GitHub Pages, Azure Static Web Apps — all free options)
  2. Download the Teams manifest package below
  3. In Teams, go to Apps → Manage your apps → Upload an app
  4. Select "Upload a custom app" and choose the ZIP file
  5. The app will appear in your personal apps sidebar
